Several forms must be submitted to process a potential student for enrollment. The documents listed below can be downloaded, printed, and submitted to the school office for processing. Contact the office at 207.865.4747 for assistance in completing the required forms:
Application for admission
Marketing Release
Medical/Physical Form is required for all new students, and students entering grades K, 1, 4, 7, and 9
Permission to Treat form
The following specific forms are applicable ONLY to Dormitory or transfer students:
Dormitory application for Girl's and Boy's Residence Hall living
Transcript Request for new students
References for new students
